Courtney Forman, MSN, PMHNP-BC, FNP-C
Nurse Practitioner
Courtney Forman is a dual certified Family Nurse Practitioner (FNP) as well as Psychiatric Mental Health Nurse Practitioner (PMHNP). She began working with In Home Medical Group (IHMG) in 2016. She obtained her Bachelor of Science in Nursing degree with a minor in Intercultural Studies from Indiana Wesleyan University in 2011. Courtney then went to get her Master of Science in Nursing degree with a focus on family practice from Northern Illinois University with FNP certification through the American Academy of Nurse Practitioners. In 2021 she completed a post master's certificate at University of St. Francis and completed PMHNP certification through the American Nursing Credentialing Center. She has completed training in alternative therapies in adult and family practice. Courtney has previous experience within the Adult Progressive Care Unit, Pediatric/Neonatal Intensive Care Unit and Pediatrics. Additional patient care populations include Geriatric Psychiatry and Subacute Adult Rehabilitation. Courtney enjoys working with children and families as well as adults to create care plans that include a variety of tools including counseling services and medication management to improve mood and promote best quality of life. Courtney joined Glenwood Center in April, 2018, as our psychiatric advanced practice nurse and will be psychiatrically treating children, adolescents and adults.
