Sarah Kohl, LCSW

Sarah Kohl is a Licensed Clinical Social Worker and Certified Hospice and Palliative Care Social Worker. She received a Bachelor of Science in Education from Rockford University and a Master of Social Work from the University of New England. Sarah was a teaching tennis professional and coach from 1991-2015 before she became a licensed social worker in 2015 and a licensed clinical social worker in 2017. She has been a hospice social worker and bereavement counselor for children, teens and adults, for the past 10 years. Sarah has advanced training and experience counseling clients with dementia; as well as supportive counseling for families, as they navigate the challenges and complexities of cognitive and physical decline.

Sarah implements a strength-based and client-centered approach that honors and respects the inherent dignity and worth of every human being. She incorporates evidence-based practices and customized modalities, such as cognitive-behavioral therapy, solution-focused therapy, grief therapy, acceptance and commitment therapy, narrative therapy, compassion–focused therapy, music therapy and mindfulness, to empower and promote the client’s best quality of life.

Sarah will offer in-home assessments and therapy sessions to clients in Winnebago and Boone County. She is committed to providing access to therapy for underserved populations, such as clients who are homebound (wherever home may be), those who lack transportation, and individuals/families who prefer out-of-office visits. Sarah feels that building a strong therapeutic alliance, based on collaboration, trust, empathetic understanding, inclusivity, authenticity and unconditional positive regard, is essential to creating a safe environment, where clients can achieve growth and optimal therapeutic outcomes.

Sarah offers therapy for clients dealing with issues such as, but not limited to:

  • Depression
  • Anxiety
  • Adjusting to chronic illness and complex medical issues
  • Anticipatory Grief and loss
  • Life transitions and stressors
  • Guidance with hospice, palliative care, advanced care planning, in-home caregiving, placement options, or other needed resources
  • Dementia and cognitive decline
  • Caregiver fatigue and stress/burnout
  • Trauma
  • Unhealthy coping skills /self-defeating thoughts and behaviors
